ASSISTED LIVING
We refer to our model of Assisted Living as “Catered Living” because we start with an individual plan of care for each person. We assess your physical and cognitive needs to offer the appropriate level of assistance.

With safety and individuality at top-of-mind, Porter Hills Village works with Teepa Snow’s Positive Approach to Care® (PAC) model and is recognized as one of only a handful of “participating” PAC organizations nationwide. This design helps each resident maintain a sense of individuality and purpose.
SKILLED NURSING
Residents in need of higher levels of care can take advantage of the best Skilled Nursing in Grand Rapids. Through both our Health Center and The Green House® Homes we offer a dedicated team of expert caregivers serving each home with around-the-clock compassionate care.
Routine Appointments Delivered Right on Campus
At Porter Hills Village, residents benefit from the convenience of 360care, which brings routine healthcare services directly to campus. Through this program, residents can schedule on-site appointments for primary care, dental, podiatry, optometry, and audiology, eliminating the need to travel off-site for many routine visits. 360care makes it easier to stay on top of wellness needs while enjoying the comfort and familiarity of home.
On-site Specialized Care for Complex Age-Related Needs
Through our partnership with Corewell Health Geriatrics, residents have direct access to on-site specialized care for complex conditions, including Alzheimer's disease, dementia, arthritis, heart, and lung disease, fall risk, and other age-related concerns. With a team-based approach, Corewell Health coordinates seamlessly with our services - delivering convenience, continuity, and peace of mind right here on campus.
